No purchase necessary to win. Participants may enter only once by submitting the contest entry form; any duplicate entries will be deleted. Entry period ends at 11:59pm Eastern Time on ___ (contest closing date). All prizes are non-transferable. Odds of winning are dependent on the number of entries received. Winners will be randomly selected and notified on or about ___ and will be announced on the X-Ray Lady Facebook page or other means once the winners are successfully notified.
